About the online course
The course will address the problems faced by any specialist in endodontics - perforations, tooth extraction or retention, resorption, etc. It's not important how good and perfect you are while doing an Endotontic treatment... sooner or later, you'll have to face a iatrogenic error. During the webinar will be demonstrated techniques to manage iatrogenic damages like floor perforations and root perforations. Step by step protocol for fixing clinically iatrogenic errors will be discussed. We will learn how to reduce the time spent on different procedures, whether there is the correct sequence of use of irrigation solutions, their concentration, activation and chemical component to achieve maximum effect in a short time. You will learn the factors for guaranteed long-term success.

  • He patented in 2005 the ProTrain (Endodontic System for Training) and right now develops new products and technologies for companies.
  • Recently he has started a partnership with AlmaPlasma,an Engineering group in Bologna.

Lesson 1
"Broken files"

  • What happened when we break a file? Are we able to remove or bypass it or should we have to consider this procedure as an impossible challenge?
  • The instrument separation is one of the most unpleasant occurrences and several techniques have been developed to facilitate the removal of the fragments; however, they generally require specific devices that not always are available to the clinician. For this reason a strategical approach must be considered and a correct analysis of the clinical situation is the most important thing to do.
  • During this webinar we will discuss the latest approaches in:

1) How to recognize a broken file before starting a retreatment;
2) The procedural decision making;
3) How to bypass a broken file in 3 steps;
4) How to remove a broken file;
5) How to avoid iatrogenic errors.
Lesson 2
"Internal and external resorptions: the challenge continues"

  • Extract or maintain? This is the eternal dilemma. As Endodontists we have to try to save teeth as much as we can. One of the biggest challenge is represented by Internal and external resorptions.
  • Resorption is considered to be internal if the original site of the resorption starts in the pulp and external if the original site is the periodontal ligament.In most cases diagnosis is simple but occasionally it may be difficult to differentiate between internal and external resorption. In internal resorption the outline of the canal is interrupted and usually appears as a smooth bulge. Internal resorption is often difficult to detect in posterior teeth and may be seen only after root treatment has been completed.
  • External resorption originates in the PDL and is recognized by an irregular radiolucent area overlying the root canal; the canal outline remains visible and intact. Sometimes external resorption is not easy to diagnose from the radiograph when the canal outline is indistinct.
  • During this webinar Dr Tonini will show step by step how to make a proper clinical diagnosis, how treat easy and difficult resorptions and if the Clinical Situation is proper or not for a treatment.

Lesson 3
"How to manage iatrogenic errors"

  • It's not important how good and perfect you are while doing an Endotontic treatment, sooner or later, you'll have to face a iatrogenic error.
  • It could be a mistake you did, it could be a mistake someone else did, but anyway you have to find way for solving and fixing it.
  • Typical iatrogenic damages are floor perforations and root perforations A perforation may occur at various stages of treatment, including access cavity preparation, calcified and curved canal negotiation and during post space preparation.
  • During this webinar Dr Tonini will explain how to manage in a predictable way all this difficult situations and will show step by step with videos ho to fix clinically iatrogenic errors.

Lesson 4
"Debridement of the root canal system: the challenge continues"

  • Debridement of the root canal by rotary files and a proper irrigation is considered the most important single factor for ensuring long term success.
  • In clinical practice, shaping is usually the most time consuming and technically demanding element of the entire treatment. Thanks of the introduction of new heat treated alloys root canal instrumentation has been optimized and time required for shaping mechanically has been drastically reduced. But still one point is missing for completing debridement: the cleaning protocol. Endoultra has been recently introduced and it is the first ultrasonic device with a cordless design able to generate cavitation and acoustic streaming inside root canal system. It means that Endoultra is able to remove efficiently smear layer, Biofilm and bacteria from the endodontic space.
  • In Endodontics, the cleaning of root canal system, is regarded by many clinicians as being the most important part of it, in order to expect a predictable successful outcome. The true goal of cleaning in endodontics is to remove all the content of root canal and ultrasonic activation is able to solve this matter. Another aspect is that, in the modern Endodontics a properly irrigation protocol takes the main part of root canal treatment because irrigants need time to maximize their effectiveness during disinfection process.
