Despite the efforts of oral health providers, caries continues to be one of the most prevalent chronic diseases in the world among all ages. Caries often starts at a young age, caused by disbalance in the dental biofilm which is influenced by self-care habits, such as oral hygiene and diet. Caries is currently considered a behavioural disease.
The habits of self-care depend on different factors: family functioning, stress levels, self-efficacy of the parents, parental oral health beliefs and behaviours. When oral health providers treat the symptoms of caries in the form of restoration, without approaching the causes of the disease, high relapse rates can be observed. A paradigm shift is needed in the provision of oral health care to prioritize the causative treatment.
Currently attention in dentistry is shifting towards behavioural modification techniques and research shows promising results of Motivational Interviewing in oral health. For example, the new evidence based Dutch Guidelines for Caries Treatment in Youth advice to use Motivational Interviewing as the basis of caries treatment.
Motivational interviewing is a patient-centered communication style with the patient or parent about their own change, where their own reasons for change are being explored and ambivalence is being resolved. Motivational interviewing has been developed by W.R. Miller and S. Rollnick and has been successful in treatment of many lifestyle related medical conditions, such as addictions, diabetes, obesity, oral diseases and other. The mindset of the health care provider together with the techniques form the basis of this facilitative style of communication.
